Melanoma: Your Visit
General Information
Location
The clinic is located at the Brown Cancer Center, 529 S. Jackson
Street in Louisville. For directions to the Center, please refer
to the pocket of your patient information packet.
Appointments
To ensure you have an appointment that is most convenient for you,
please call as early as possible to schedule that appointment. If
you need to cancel or change your appointment, please notify our
office as soon as possible (at least 24 hours in advance), giving
your time and date of appointment.
Your Visit
Patient Registration
When you arrive at the Brown Cancer Center, check in at the Registration
Desk. Our friendly staff will give you directions to where you need
to go after your register.
If another physician has referred you to the Melanoma Clinic, we
will assume you want that physician to receive a report of our findings
and recommendations. Unless otherwise requested, we'll promptly
forward such reports following your visit. Please be assured that
we keep your medical records strictly confidential and will not
disclose them without your permission.
What to bring
Contact your physician’s office prior to your appointment
to determine what to bring with you to your appointment.
Fees for Services
You may incur charges from each clinical specialist involved in
your treatment and care. If you have questions about the purpose
or anticipated costs of a procedure, our staff will be more than
happy to fully discuss all aspects with you prior to treatment.
Insurance
As a professional medical care provider, the Multidisciplinary Melanoma
Clinic does not accept the responsibility of negotiating claims
with insurance companies or other persons. The patient is responsible
to the Melanoma Clinic for payment of accounts within a reasonable
time, regardless of the status of a claim. In circumstances where
a claim is pending or when treatment will be for an extended period
of time, we recommend arranging a payment plan.
